About the Agency

An all-service VA Medical Center in an all-season recreational getaway!  A Clinical Pharmacy Specialist is needed in Augusta Maine. Working in the Acute Care and Medicine departments of the Medical Center, the CPS will perform duties of a clinical and teaching nature, including direct and indirect patient care, routinely functioning as a team member with minimal supervision in the medical/surgical acute care practice areas.

It's a modern 67 bed facility with general medical, surgical, intermediate and mental health beds, as well as 40 Skilled-Longer Stay beds, a 25-bed Dementia Unit, and a hospice unit. The incumbent is responsible for planning, developing and maintaining effective clinical pharmacy programs by implementing rational cost effective drug therapy to improve patient outcomes in the medical/surgical practice areas.  (S)He must be capable of using professional judgment and clinical expertise to solve complex problems and to provide clinical information to providers and Veterans.  (S)He also serves as a resource to this team for patients receiving antibacterial, antiviral and antifungal therapy. In-depth knowledge and experience of Infectious Disease is required.

Clinical:

The incumbent provides care based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in the assigned areas (i.e. adult and geriatric populations).

A. Interfaces with the inpatient medical/surgical provider in the design of the veteran’s drug treatment regimen.

B. Participates in inpatient rounds with medical/surgical services.

C. Serves as an authoritative information source on drugs and their utilization in therapy.

D. Serves as the Antimicrobial Stewardship Champion as required by VHA Directive on Antimicrobial Stewardship.

E. Evaluates the drug literature by analysis of experimental design and evaluation, in order to compare and contrast therapeutic regimens and roles for new drugs as it applies to patient care.

F. Serves as an authority on pharmaceutical care in specialized and routine areas of clinical pharmacy practice for medicine, nursing, and pharmacy staff.

G. Responsible for daily performance and oversight of provider-consulted, clinical pharmacy pharmacokinetic monitoring program for acute care wards.

H. Responsible for daily performance of clinical pharmacy anticoagulation monitoring program for inpatient wards as assigned.

I. Provides consultation on selection of appropriate dosage regimens as follow-up when non-therapeutic drug levels are obtained for ambulatory care and inpatient medical/surgical patients.

J. Assesses patients on parenteral and enteral nutrition.  In conjunction with nutrition, parental formulations are reviewed for appropriateness based on laboratory parameters.

K. Works collaboratively with the inpatient pharmacy staff to ensure timely and appropriate medication administration.   


Administrative:

A. Reviews electronically-submitted non-formulary drug requests (NDR) utilizing National VA PBM Criteria for Use.

B. Participates and/or supports Pharmacy Service research efforts

C. Completes all encounters, charting and documentation including scheduling, reviewing consults and finishing encounters.
D. Participates and/or supports Pharmacy Service-initiated cost/benefit programs in support of VA Maine Healthcare's efforts at maintaining stewardship of resources

E. The incumbent will be expected to provide follow-up on reported adverse drug events in the inpatient medical/surgical patient care areas or other areas within the medical center when necessary.  (S)he is be expected to submit completed adverse drug event report forms to the Clinical Coordinator of Pharmacy Service as part of the follow-up process.

F. Assists in conducting and reporting internal audits as required.  Maintains all pertinent professional and administrative records as necessary.  (S)He assists in data collection for MUE and participates in quality assurance.


Education:

A. Assumes an active role in the overall pharmacy-training program including pharmacy students, pharmacy residents, professional and non-professional staff members.

B. Attends and actively participates in clinical pharmacy activities, i.e., staff development, clinical pharmacy meetings, journal club.

C. When selected to precept Pharmacy Residents, participates fully in providing feedback, evaluation and documentation of Pharmacy Residents utilizing ResiTrak software.

D. Acts as a source of drug information for health professionals in the inpatient acute care wards.

Responsible for keeping abreast of changes in the Pharmacy Service and to maintain a general knowledge of VA, DEA and JCAHO regulations.

BASIC REQUIREMENTS:

- Citizenship. U.S. Citizen. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

- Education. Must be a graduate of a PharmD degree program in Pharmacy from an American Council on Pharmaceutical Education (ACPE) accredited college or university.

- Licensure. Must have a full, current and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States or District of Columbia.

- English Language Proficiency. Must be proficient in spoken and written English.

Preferred Experience: Post-graduate year one (PGY1) residency training or equivalent experience, Board Certification, experience in acute care, anticoagulation management, pharmacokinetic monitoring, active participation in residency training programs, clerkships, internships, and staff development programs.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

GRADE DETERMINATIONS:

GS-13

At the GS-13 level, in addition to the basic requirements, candidates must have an equivalent of 1 year of creditable experience equivalent to the next lower grade level GS-12, which is directly related to the position to be filled.

Experience: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice; communication orally and in writing with patients and healthcare staff; knowledge of laws, regulations and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security; skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

NOTE: Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek.

In addition to meeting the basic requirements, pharmacists at the GS-13 level must demonstrate the following Knowledge, Skills and Abilities (KSAs):

1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.

2. Expert understanding of regulatory and quality standards for their program area.

3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area of clinical expertise.

4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.

5. Advanced skill in monitoring and assessing the outcome for drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
