As a condition of employment with the Durham VAMC, the selectee will have to obtain/maintain a license with the State Of North Carolina as a Nurse Pratitioner (NP) in the specialty which they are being appointed to. Must be proficient in written and spoken EnglishSelective Service Registration is required for males born after 12/31/1959Tour of Duty:Monday-Friday, 8 hour tours, occasional weekend coverageThe CLC Hospice is a 10-bed unit providing pain control, symptom management, end of life care and bereavement support to the residents and family of the imminently dying. The program is designed to provide optimal comfort care to Veteran and meets the mission, vision and values of the VAMC and nursing division. The acuity ranges from comfort care to some critical care type modalities when needed to control pain. The length of stay for veterans in the unit may range from a few hours to 30 days or occasionally longer. Veterans are accepted for Hospice care from the Emergency Department, ICU, medical/surgical wards, home and community facilities. The CLC Hospice Unit has an interdisciplinary team that includes staffing representing Nursing, Chaplain, Social Work, Pharmacy, Dietary, Nurse Practitioners, and Physicians. The interdisciplinary team meets to plan care and modify treatment according to needs of Veteran and family. End of life care Veterans may require IV push medication, PCA pumps, epidural analgesia, BiPap, CPap, and management of delirium. The successful candidate will be responsible for:· Primary care of inpatients on the Hospice/Palliative Care Unit.· Conducting admission history and physical assessment. Writing admission notes and discharge summaries.· Writing orders for patient care, evaluation and treatment plans in accordance with the individuals Scope of Practice and under the authority · of the Medical Director of the Hospice Unit or their designee.· Prescribing and monitoring medications as allowed by the Durham VAMC under the guidelines of the NP Scope of Practice.· Ordering and interpreting medical tests and x-rays · Performing procedures to include minor surgical procedures (suture removal, I&D, removal of foreign bodies etc.).· Counseling patient/family for terminal and end of life care. · Education Champion for End of Life Care for Veterans. Responsibilities include providing regular education offering for Nursing Staff and· Medical Center personnel.· Attending Committee meetings, Process Action teams, etc to represent the Hospice/Palliative Care service as directed by the Medical · Director.· Coordinating referrals, consults, and discharge activities.· Screening and evaluation of patients referred to Hospice/Palliative Care· Facilitating response/action to patient inquiries regarding care and treatment · Active member of the Palliative Care Consult Team.· Other administrative duties as assigned by the Medical Director of Hospice/Palliative Care and the ACNS for CLC.The Scope of Practice of all registered nurses in all areas of clinical practice is defined in the four (4) dimensions outlined in the Nurse Qualification Standard, and is specific to the Grade:1.PRACTICE: Provides leadership in the application of the nursing process to patient care, organizational processes and/or systems, improving outcomes at the program or service level.ETHICS:Provides leadership in identifying and addressing ethical issues which impact patients, staff, including initiating and participating in ethics consultations. RESOURCE UTILIZATION: Manages program resources (financial, human, material or informational) to facilitate safe, effective, efficient care. 2.PROFESSIONAL DEVELOPMENT (Education/Career Development, Performance). COLLEGIALITY: Makes sustained contributions to healthcare by sharing knowledge and advanced skills internal and external to the healthcare system. The NP will coach colleagues in team building.4.SCIENTIFIC INQUIRY (Quality of Care, Research).QUALITY OF CARE: Initiates interdisciplinary projects to improve organizational performance. RESEARCH: Collaborates with others in research activities to validate and/or improve care.To qualify for this position, applicants must meet all requirements by the closing date of this announcement. Graduate of a school of professional nursing approved by the appropriate State agency and accredited by the National League of Nursing at the time the program was completed by the applicant. In cases of graduates of foreign schools of professional nursing, possession of a current, full active and unrestricted registration will meet the requirement of graduation from an approved school of professional training.Master’s degree from a program accredited by one of following: (1) The National League for Nursing Accrediting Commission (NLNAC), or (2) The Commission on Collegiate Nursing Education (CCNE), an accrediting arm of the American Association of Colleges of Nursing (AACN).Licensure. Current, full and unrestricted licensure as a Registered Nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico). Physical Requirements: The Nurse Practitioner is required to meet the physical standards as outlined on the OF 178 Certificate of Medical Examination (Attachment A). The position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring Veterans and objects may be required Staff may be required to don protective equipment in isolation situations or operative/invasive procedures. RN is responsible for the reprocessing of reusable medical equipment, according to the medical center policies. They are required to use available safe patient handling lifting devices in all clinical areas. They may also be exposed to Veterans who are combative, secondary to delirium, dementia, or psychiatric disorders.
